We have an exciting opportunity within Specialist Children Services for a Band 6 Community Children’s Nurse. Additional Support for Learning Schools to join our team. Under the direction of the Clinical Nurse Specialist / Team Lead the post holder will have responsibility for assessment, planning, implementing and evaluating programmes of care for their own caseload for patients experiencing disability and developing the role of the Specialist Children’s Services Nurse in all aspects of the disability pathway. Under the direction of the Clinical Nurse Specialist / Team Lead the post holder will have responsibility for assessment, planning, implementing and evaluating programmes of care for their own caseload for patients experiencing disability and developing the role of the Specialist Children’s Services Nurse in all aspects of the disability pathway. The post holder will be expected to provide an accessible, appropriate, high quality, culturally sensitive, safe, patient-centred and effective service in the community by working in partnership with children young people and families, care providers and other agencies.
Ideally have experience of working with children and young people from age 2-19 years, with Complex Medical and Social Needs. Who has experience of caseload management and can prioritise their allocated workload and is able to delegate appropriately .
We deliver Competency Based training packages to non health staff therefore experience in clinical interventions and assessment of care needs is desirable.
We carry out Nurse Led Assessments and deliver the school aged Immunisation Programme.
Full induction will be given and upskilling in clinical interventions as required.
